1) I understand that the use of the facilities, activities or programs at Dance Cavise Inc. involve risk and danger of physical stress, strain, and injury to me/my child and I admit that I am freely assuming all the risks involved. Moreover, I knowingly state that I/my child am in good physical condition and do not now have any impairment, disability, sickness, or disease which would prevent me from using any of the facilities, activities or programs at the studio.

2) In consideration of the foregoing, I promise and agree to forever hold harmless, release, acquit and discharge Dance Cavise Inc., its Owners and Employees, of and from any and all actions, causes of action, claims, demands, damages, costs, loss of service, expenses and compensation on account of, or in any way growing out of any and all personal injuries and property damage resulting from my participation in any activities or programs herewith.

3) I also agree and understand that the protection of my personal property is my individual responsibility and that Dance Cavise Inc. shall not be liable for the loss, theft of, or damage to any of my personal property.

4) I understand and agree that I am solely and individually liable and responsible for any damages which either I myself or my guest causes to the Studio premises. Should any such damage occur irrespective of whether it is the result of an intentional act or negligence, I agree and promise to pay back and indemnify Dance Cavise Inc. for all such damage.

5) I understand that any/all photos and or videos taken of my child may be used for publicity purposes for Dance Cavise Inc. without compensation to my child/myself.

6) By placing my signature on this agreement, I represent and take full responsibility for my child/myself, that I am over 18 years of age, I have read, and fully understand, all Dance Cavise Inc. policies and I agree to abide.

7) REFUNDS: One half tuition deposit is required to register, but it must be paid in full at the time of registration to receive applicable discounted rates. Dance Cavise Inc. will refund 75% tuition prior to May 1st with no refunds, deductions or credits after that date for any reason. All summer tuition balances are due by June 1st.
