In consideration of being allowed to participate in any Dance Cavise Inc. related program, including but not limited to class/activities at 273 Halstead Avenue, Mamaroneck NY, class/activities at any other location, and/ or taking class online, I agree to the following:

1) I understand that my participation in any activity or program at Dance Cavise Inc. involve risk and danger of physical stress, strain, and injury to me and I admit that I am freely assuming all the risks involved. Moreover, I knowingly state that I am in good physical condition and do not now have any impairment, disability, sickness, or disease which would prevent me from using any of the facilities, activities or online programs with Dance Cavise Inc.

2) In consideration of the foregoing, I promise and agree to forever hold harmless, release, acquit and discharge Dance Cavise Inc., its Owners and Employees, of and from any and all actions, causes of action, claims, demands, damages, costs, loss of service, expenses and compensation on account of, or in any way growing out of any and all personal injuries and property damage resulting from my participation in any activities or programs, regardless of where such programs are located, herewith.

3) I also agree and understand that the protection of my personal property is my individual responsibility and that Dance Cavise Inc. personnel shall not be liable for the loss, or theft of, or damage to any of my personal property.

4) I understand and agree that I am solely and individually liable and responsible for any damages which either I myself or my guest causes to the Studio premises. Should any such damage occur irrespective of whether it is the result of an intentional act or negligence, I agree and promise to pay back and indemnify Dance Cavise Inc. for all such damage.

5) I understand that any/all photos and or videos taken of my child may be used for publicity purposes for Dance Cavise Inc. without compensation to my children or me.

6) I understand that refunds/credits are only applicable up to one week from my start date. After that time, no refunds/credits/deductions will be given. I understand that $125.00 of my tuition deposit and an additional $75.00 administrative fee are non-refundable. I understand that no exceptions will be made to the refund/credit policy for any reason.

7) I understand that Dance Cavise Inc. is not responsible for any issues related to the internet including, but not limited to, the connectivity of online classes, or any result in the lack of communication regarding studio events or policies.

8) I also acknowledge and am giving my consent that appropriate physical contact with my child is needed for correctional purposes. I will hold harmless any representative of Dance Cavise Inc. in this situation.

9) By placing my signature on this agreement, I represent and take full responsibility for my child/myself, that I am over 18 years of age, and agree to indemnify, defend, and hold harmless Dance Cavise Inc. for any liabilities, loses, damages, and costs arising from me or my child’s participation in the described activity, and agree to abide and comply by all terms herewith.
